This Notice describes how HBOTT.com protects health information collected through Find a Clinic and Ask the Oracle, and how we adhere to electronic PHI (ePHI) protection standards as a Wellness Modality platform.

This HIPAA Compliance Notice ("Notice") describes how health information about you may be used, disclosed, and protected by HBOTT.com and our affiliated providers. It applies to protected health information (PHI) and electronic PHI (ePHI) we create, receive, or maintain in connection with hyperbaric oxygen and related wellness modality services, including information collected through our Find a Clinic and Ask the Oracle features. Please review it carefully.

HBOTT.com as a Wellness Modality Platform; ePHI Standards

HBOTT.com operates as a Wellness Modality platform that facilitates discovery of hyperbaric oxygen therapy (HBOTT) providers, education, and related services. We adhere to electronic PHI (ePHI) protection standards consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the HIPAA Security Rule where we create, receive, maintain, or transmit ePHI. This includes administrative, physical, and technical safeguards to protect the confidentiality, integrity, and availability of ePHI. We do not use or disclose your PHI or ePHI except as described in this Notice or as otherwise permitted or required by law.

Protection of Health Information: Find a Clinic and Ask the Oracle

Find a Clinic: When you use the "Find a Clinic" (or similar directory) feature, we may collect location data (e.g., ZIP code, city, state), search preferences, and any optional information you choose to submit (e.g., condition or inquiry details). This information is used solely to help you discover providers and is protected in accordance with our ePHI safeguards. We do not share your search or location data with clinics or third parties for marketing without your consent. Data transmitted and stored in connection with Find a Clinic is encrypted and access is restricted to authorized personnel and systems necessary to operate the service.

Ask the Oracle: The "Ask the Oracle" (AI Oracle) feature allows you to submit questions or topics related to hyperbaric oxygen therapy and wellness. Responses are for informational and educational purposes only and do not constitute medical advice. Any health-related information you voluntarily provide in your queries (e.g., symptoms, conditions, or treatment history) is treated as potentially identifiable health information and is protected under our ePHI standards. We do not use Oracle conversations for marketing or share them with third parties except as necessary to operate the service, improve quality (in de-identified form where possible), or as required by law. Storage and transmission of Oracle-related data are secured consistent with this Notice.

Our Commitment

We are required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and applicable state law to maintain the privacy of your protected health information and to provide you with this Notice of our legal duties and privacy practices. We will not use or disclose your PHI except as described in this Notice or as otherwise permitted or required by law.

Medical Treatment Logs and Session Records

In the course of providing HBOTT (hyperbaric oxygen) sessions, we create and maintain medical treatment logs and related records. These may include: your name and contact information; session dates, times, and duration; pressure (ATA) and protocol parameters; your reported symptoms or condition (if provided); and clinical notes necessary for continuity of care, quality assurance, and legal/regulatory compliance. These treatment logs and records are protected health information (PHI) and are used and disclosed only as described in this Notice and as permitted by HIPAA and state law.

How We May Use and Disclose Your PHI

We may use and disclose your PHI, including information in medical treatment logs and session records, for the following purposes:

  • Treatment — To provide, coordinate, or manage your hyperbaric sessions; to communicate with your referring physician or other healthcare providers when you have authorized such communication or when permitted by law.
  • Payment — To bill and collect payment, including from you, your insurance, or a third party (e.g., lien holder, attorney) when applicable.
  • Healthcare operations — For quality improvement, training, scheduling, and other operational purposes necessary to run our practice.
  • As required by law — When required by federal, state, or local law (e.g., reporting to public health authorities, response to court order).
  • With your written authorization — For any other use or disclosure not described above, we will obtain your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already relied on it.

Your Rights

You have the right to: (1) Request access to and a copy of your PHI, including medical treatment logs and session records, subject to limited exceptions; (2) Request correction of your PHI if you believe it is inaccurate or incomplete; (3) Request restrictions on certain uses and disclosures (we are not required to agree in all cases); (4) Receive confidential communications at an alternative address or phone if you specify; (5) Receive an accounting of certain disclosures of your PHI; (6) Obtain a paper copy of this Notice upon request. Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint. Complaints to us should be submitted in writing to the contact address below.

Changes to This Notice

We may revise this Notice from time to time. The current version will be posted on HBOTT.com at this page (and at /hipaa-notice). A copy will also be available upon request at our office.
